OBJECTIVE:Studieshaveindicatedthatp53proteinaccumulationexertsanadverseeffectonthesurvivalofbreastcancerpatients;however,theprognosticvalueofp53proteinaccumulationforaromataseinhibitor(AI)resistanceinER-positivebreastcancerisuncertain.

METHODS:Theexpressionlevelofp53proteinwasdetectedbyimmunohistochemistryinprimaryearly-stageER-positivebreasttumorspecimensfrompostmenopausalbreastcancerpatientswhoreceivedfirst-lineAItreatment(letrozole,anastrozole,orexemestane)untilrelapse,andanalysiswasperformedtodeterminewhetherexpressionofp53proteinaffectedtheresponsetoendocrinetherapy.

RESULTS:Oftheinvasiveductalcarcinomas,65.4%werepositiveforp53proteinexpression.AllpatientsreceivedAItherapyasfirst-linetreatmentuntilrelapse.The5-yeardisease-freesurvivalratesinp53-positiveandp53-negativepatientswere78%and89%,respectively.Patientswithprimarybreasttumorsthathadp53proteinaccumulationshowedsignificantlymoreresistancetoAItreatment(hazardratio=1.,95%confidenceinterval=1.-2.,P=0.).

CONCLUSION:Thisstudydemonstratedthatp53proteinaccumulationwashelpfulinchoosingpatientswhomaybenefitfromAItreatmentandisaprognosticmarkerinER-positiveearly-stagebreastcancer.
